Ill health certificates for active and tier 3 members
- Medical certificate 1: current employee medical certificate (docx format, 100 KB)

Ill health certificates for deferred members
- Certificate 11: deferred pension - left employment after 31 March 2014 (docx format, 799 KB)
- Certificate 2: deferred pension - left employment after 31 March 2008 and before 1 April 2014 (docx format, 87 KB)
- Certificate 3: deferred pension - left the LGPS after 31 March 1998 and before 1 April 2008 (docx format, 790 KB)
- Certificate 4: deferred pension - left the LGPS before April 1998 (docx format, 84 KB)
Other medical certificates
You can request the following documents from Pension Services:
- Certificate 8: tier three pension in payment - leaver after 31/3/2014 requests review within 3 years of leaving
- Certificate 9: tier three pension suspended - leaver after 31/3/2014 requests review after 3 years from leaving
- Certificate 9: tier three pension suspended - leaver before 1/4/2014 requests review more than 3 years after pension stopped
- Certificate 6: request for early payment of deferred pension on health grounds for a former elected member
